• Present Your Prescription Drug Discount Card along with your prescription
to the Pharmacist at any Kmart Pharmacy, nationwide.
• The Pharmacist will enter the information into the computer, which
will then price your prescription at the negotiated rate or Kmart's Everyday
Low Price, whichever is less. You pay 100% of the lowest price.
• The CARD will help you pay the lowest price possible for your prescription.
If you find the same prescription elsewhere for less, Kmart will match
that price. That is your Kmart Low Price Guarantee.
To Order Your
Prescriptions by Mail
• Ask your Doctor to prescribe your maintenance medications for a
90-day supply.
• Complete the patient profile order form. Be sure to sign the medical
information and generic authorization box. Include your payment information.
• Be sure to write your plan code number from your discount card
in the upper right hand corner box of the patient profile mail order form.
• Mail your original prescriptions with your patient profile order
form and your payment to the address listed below:

your Refill prescription orders 14 days before you need them.
